Sample Acceptance Policy
Effective Date: March 29, 2023
AP Genomic Laboratories Inc. may not accept samples for testing if they do not meet the following criteria:
- Samples arrive with a complete Chain of Custody (COC) specifying at the minimum: client’s name and contact information; sampler’s name; sample identification and number of containers; sampling location, date and time(s); requested test methods, matrix and compound list.
- Sample containers are properly and uniquely identified to match the COC.
- Samples arrive in containers that meet the selected method(s) requirements, with appropriate preservation and within the temperature requirements.
- Sample containers have adequate amounts/volume of sample as specified by the selected method(s).
- Samples arrive to the laboratory within reasonable time to be analyzed before the holding time expiration.
If any of these requirements are not met, the samples may be put on hold and the client will be contacted for further directions.
If the sample collection time is not provided, the laboratory will use the most conservative time (12:00 AM of the date provided).
All deviations or anomalies will be described in the lab report comment section.
